Were you once a patient at Cypress Fairbanks Medical Center Hospital? Did you deliver a baby at our facility? If you need to obtain medical or other records from our hospital, please follow the procedures below.
Medical Records
To protect your privacy, Cypress Fairbanks Medical Center Hospital does not transmit confidential patient information via e-mail. To obtain copies of your records, please call our Medical Records Department at 281-897-3148 during regular business hours, Monday - Friday from 8 a.m. to 5 p.m.
In order to obtain a copy of your medical records, please complete this form. The completed form may be delivered in person to the Medical Records Department or by mail to the following address:
