To protect your privacy, Cypress Fairbanks Medical Center Hospital does not transmit confidential patient information via e-mail. To obtain copies of your records, please call our Medical Records Department at 281-897-3148 during regular business hours, Monday - Friday from 8 a.m. to 5 p.m.

In order to obtain a copy of your medical records, please complete this form. The completed form may be delivered in person to the Medical Records Department or by mail to the following address:
